He was introduced to homoeopathy in 1847, while he was living in Lockport, NY. He became the professor of Clinical Medicine when he joined the faculty at the New York Homoeopathic Medical College in 1869. He was instrumental in encouraging women to become a homoeopath and was a prolific author. With the support of Constantine Hering, Lilienthal became an editor of the 'North American Journal of Homoeopathy' from 1872-1885. His comprehensive work, Homoeopathic Therapeutics, was published in 1878. He proved several remedies including Carbolic acid and Physostigma venenosum. Lilienthal moved to San Francisco in 1887 to join his son James, who was teaching at Homoeopathic Medical College of the Pacific. Lilienthal then retired 1 year later.
