Preface

When creator made this universe he wanted all to be blessed with sound health and mental status .Ayurveda is one among such systems of medicine which helps us to lead a life in accordance with nature and to the creator.

Ayurveda, an ancient and time tested knowledge of medicine gained over trial and error over thousands of years, is a very comprehensive system; based on a holistic medicinal approach that has its roots in Vedic Culture. Ayurveda is not only a system of medicine but it also represents a way of healthy living.

Ayurveda is formed by the combination of two words viz. 'Ayur' meaning life and 'Veda' meaning knowledge. Thus, Ayurveda means the "knowledge of life". Ayurveda is grounded in metaphysics of the 'five great Elements' and stresses a balance of three Humors or Energies.

The focus of Ayurveda is to integrate and balance the body, mind, and spirit, rather than focusing on individual symptoms. Ayurveda philosophy posits people, their health, and the universe are related. We can even get miraculous results for some of the dreadful diseases through ayurveda.

In the ancient India, surgery was principally pioneered by Ayurveda. A 16thcentury French surgeon, stated that there were five reasons to perform surgery: "To eliminate that which is superfluous, restore that which has been dislocated, separate that which has been united, join that which has been divided and repair the defects of nature." As human beings in their course of evolution were primarily interested in making tools and handling it for their needs. Its not-surprising that surgical techniques too were employed quite early.

Shalya Chikitsa is a significant branch of Ayurvedic science. , its best suited, when the problem is beyond just medicinal repair. The name of the sage-physician, Susruta, is synonymous with shalya tantra. His conceptions of anatomy, physiopathology and therapeutic strategies were of unparalleled brilliance, considering the early age in the history of mankind with no supportive knowledge base preceding his era. Susrutha was the first medical man who advocated and brought into practice the dissection of human cadavers by the students of surgery.

The very idea that in 1000 BC practical anatomical study was considered important itself is noteworthy. His description of different parts of the body, muscles, bones, blood vessels, tissues, special spots of surgical importants, studies of foetus in womb and manipulating cases of obstructed lab our reflects the sound knowledge of surgeons of those days. The surgical techniques he put forth stood the test of time, all the operative manouvers carried out by the present day surgeons include one or more of these techniques only and not any thing beyond these., Susruta was the pioneer in arresting haemorrhage by apposition of the cut edges with stitches, application of styptic decoctions, by cauterisation with chemicals or heat, which the modem science adopted decades later.

Vedic knowledge was passed through cognition or by meditation. The knowledge and the technique of kshara, Agni karmas or the use of various methods of healing, prevention, longevity and surgery came through Divine revelation. These revelations were transcribed orally among the disciplines of dhanwanthari sampradaya like Aupadhanava, Aurabhra, Susruta and Paushkalavata etc who wrote treatises on Salyatantra (surgery) which became the sources of the later works on this subject. Then earliest scripts would have been written on perishable materials such as Taalpatra and were passed on to the next generation, and now that "divine science developed into a statutory, recognized medical system of health care. The Central Council of Indian Medicine (CCIM) governs and recommends policies for the research and development of the system. In India, practitioners in Ayurveda undergo an 5 1/2 years of training including 1 year of internship wherein they earn the professional doctorate degree of Bachelor of Ayurveda Medicine and Surgery [B.A.MS.].

I have kept changes to the wording of the second-edition material to a minimum. Obvious spelling and typographical errors have been corrected, and the entire text has been edited and changed as per the new ccim syllabus (2013). This book 'Outline or Salyatantra' third edition strictly covers word to word syllabus by the Central Council of Indian Medicine for BAMS course. I am in every way tried my level best to make this vast subject of ayurvedic surgery in a capsule form and a bit easy for the students. Relevant portions from modern surgery were also included and its also made in an examination point of view.

In preparing this new edition I have benefited from discussions with Dr. N.V. Sreevaths Principal and my colleagues of Rajiv Gandhi Ayurveda Medical College, Mahe. Finally, I am indebted to the Ayurvedic students and beneficiaries of this book.

Forward

It gives me immense pleasure and pride to introduce "outline of salyatantra" a textbook on salyatantra authored by Dr. Syyed mohammed jalaludheen MS (Ay) Ph.D (Ay), who is one of my beloved students. Even during his academic days, he had impressed me with his hard work, honesty and dedication to the subject. The present work definitely reflects these qualities of the author.

The demand for a comprehensive textbook on salyatantra confirming to the syllabus stipulated by the CCIM for the same has been a long standing one. Though several text books have come out over the years on the lines of the syllabus, they have not found wide acceptance in Kerala. This is due to the fact that the practice of salyatantra in Kerala is quite different from than in other parts of India, where modem surgical practice is more prevalent. So most of the textbooks available now have missed out hugely in accommodating the genuine practice of salyatantra popular in Kerala, such as marma therapy, the practice of traditional bone setting, management of anorectal conditions, para surgical techniques such as jalookavacharana, agnikarma, ksharakarma etc.

These problems have been overcome significantly in the present text, with regards to all the possible topics. The author has tried to bring to the fore all the highlights of surgical practice in Kerala. At the same time he has diligently incorporated even the latest advances of the modem surgical practice. This approach of incorporating both the Ayurvedic as well as the Modem outlook of every possible surgical condition has endowed the text with authority and authenticity which few others can claim of.

The present work" outlook of salyatantra" covers the topics prescribed for the study of salyatantra as per the CCIM syllabus subsequent volume to the same is learned to have been envisaged by the author accommodating the syllabus recommended for salyatantra. The style of presentation of the present text is lucid and the language simple, so that any reader of this text be it a student, research person or practitioner would find it immensely useful and hugely informative. The author has successfully incorporated all the relevant information regarding every topic without losing focus. This has helped in enhancing the readability of the text without compromising on the authenticity.

I definitely feel that this work would contribute significantly in the study of salyatantra and in the popularization of its practice. I wholeheartedly congratulate the author for his relentless effort and enthusiasm that has gone into the shaping of this book; I also wish him success for his subsequent endeavor.
